		Developing a Systematic Approach for Transitioning Patients to College Mental Health Services
	
					
	For youth with pre-existing mental health issues, the transition from high school to college presents particular challenges. Identifying at-risk youth early, and encouraging patients and parents to think about potential challenges in advance of the college application process, can help. Participants learn to use a comprehensive transition toolkit to empower patients and families in their decision-making. This Workshop shares information on post-secondary education options, educational laws, privacy laws, and campus resources; ways to empower parents and patients to advocate for themselves; ways to encourage adolescents to gradually take on increased responsibilities for self-care; and best practices for helping families navigate the transition.
